Dark circles are one of the most commonly raised concerns in aesthetic consultation, and one of the most frequently mistreated, because the term covers several distinct underlying causes that do not all respond to the same treatment. Before recommending filler, it is worth being clear about which mechanism is actually producing the appearance in front of you.
Structural Causes: Volume and Bone Anatomy
Everyone has some natural hollowing around the eyes, but the degree varies considerably by anatomy. Some patients are born with comparatively little fat volume in the periorbital area, or a more prominent orbital rim, which produces a visible shadow independent of age or tiredness. - Allergy and irritation. Chronic rubbing or allergic shiners can also darken the area through a different mechanism again.
Matching the Cause to the Treatment
Filler is designed to add volume from beneath the skin, which lifts thin overlying tissue away from the shadow-casting structures below. That makes it genuinely effective where hollowing is the dominant issue, but it will not lighten pigment or reduce the visibility of superficial blood vessels — recommending filler for a primarily pigment-driven case is likely to disappoint the patient regardless of technique. A short assessment distinguishing hollowing from discolouration should always precede a treatment recommendation.
