Fat-Dissolving Injections: Mechanism, Candidacy and Treatment Course

Educational reading for licensed practitioners — not clinical or injection guidance.

Injection lipolysis has become a mainstay of non-surgical body-contouring menus, offering patients a way to reduce localised pockets of stubborn fat without the surgery and recovery time associated with liposuction. The terminology around it is inconsistent — "fat-dissolving injections", "injection lipolysis", "intralipotherapy" and "mesotherapy" are often used interchangeably — which makes it worth clarifying what each actually refers to before comparing products. This overview is educational and is not an injection protocol.

Sorting Out the Terminology

Lipolysis is simply the biological process by which fat cells break down. Injection lipolysis is the clinical procedure of triggering that process via injection, and intralipotherapy is essentially a synonym for the same thing. Mesotherapy is a broader technique, originally developed in France for a range of dermatological and circulatory concerns, involving superficial injection of various substances — vitamins, amino acids, minerals, and, among other things, fat-dissolving agents. Because fat-dissolving substances are one of the things mesotherapy can deliver, the two terms have become blurred in casual use, but mesotherapy is the wider technique and injection lipolysis is one specific application of it.

How the Main Active Ingredients Work

Two ingredients account for most of the injectable lipolysis market:

  • Deoxycholic acid, a bile salt the body naturally produces to help digest fat. When injected into subcutaneous fat, it disrupts the fat cell membrane, causing the cell to break down; the resulting lipid content is cleared through the body's normal metabolic and lymphatic pathways. It is the active ingredient in Aqualyx and in standalone deoxycholic acid preparations.
  • Phosphatidylcholine, derived from soy lecithin and also naturally present in the liver, works by a related but distinct mechanism, destabilising the fat cell membrane. It is often formulated alongside carnitine, which supports the metabolism of the released lipids.

Both are naturally occurring in the body, which supports their biocompatibility and relatively favourable safety profile compared with less targeted approaches.

Typical Treatment Areas

Submental fat — the area under the chin — is the most frequently treated site and the one with the most robust regulatory approval history, but the same technique is applied off-label across the flanks, abdomen, upper arms, inner and outer thighs, and back. It is worth noting explicitly to patients that these are localised-fat treatments, not weight-loss treatments, and are not appropriate for patients with obesity or those seeking overall weight reduction.

How This Differs From Liposuction

Injection lipolysis is an ambulatory, needle-based procedure that can typically be completed within thirty minutes and involves no incisions, no fat extraction, and minimal downtime beyond a few days of swelling. Liposuction, by contrast, is a surgical procedure performed under anaesthesia, offering a faster, more dramatic single-session result but with a correspondingly higher risk profile and longer recovery. Framing injection lipolysis as a lower-magnitude, lower-risk alternative — rather than a direct substitute for surgical fat removal — helps set accurate expectations.

Course Length and Timeline

Results build gradually and require a course rather than a single session; most protocols involve two to six sessions, spaced three to four weeks apart, depending on the area and the amount of fat being targeted. Because the treated fat is cleared through normal metabolic processes, visible change is not immediate — patients are typically advised to expect a noticeable reduction only from around four weeks after a session, once clearance has taken place. As fat volume reduces, the skin's own elasticity is what determines how well the area re-drapes, which is a relevant consideration when assessing candidates with looser skin.

Candidate Considerations

Good candidates have localised, diet-and-exercise-resistant fat rather than generalised excess weight, are in good general health, and understand that results are cumulative across a course. As with any injectable, a careful health history should rule out relevant contraindications before treatment begins.

The Takeaway

Injection lipolysis gives clinics a genuinely non-surgical option for localised fat reduction, built on well-understood, naturally occurring active ingredients. Setting the terminology straight for patients — and being clear that this is a course-based, localised-fat treatment rather than a weight-loss solution — does most of the work in managing expectations.
