Hyaluronidase, an enzyme that breaks down hyaluronic acid, is the tool that makes reversal possible. Any clinic offering HA filler should hold hyaluronidase in stock and understand how it behaves, even if it is used only occasionally. This overview answers the questions that come up most often and is educational rather than an injection protocol.
What Hyaluronidase Does — and Does Not Do
Hyaluronidase acts specifically on hyaluronic acid. It has no effect on fillers formulated from other substances, such as polymethylmethacrylate (PMMA) or polyacrylamide, which break down through entirely different, much slower biological pathways. This matters at consultation: before reaching for hyaluronidase, it is worth confirming, where possible, what the original filler actually was.
How Quickly Does It Work?
Visible softening of treated HA typically begins within minutes and continues over the following 24 hours, though the full picture can take longer to assess in practice. The main variable is the degree of cross-linking in the original filler. Manufacturers cross-link the sugar chains in HA to make it more resistant to breakdown and to give it a firmer, longer-lasting gel structure — the same property that extends a filler's longevity in the skin also makes it more resistant to hyaluronidase. Densely cross-linked, structural fillers may need more than one session to fully dissolve, whereas softer, less cross-linked formulations typically respond faster.
When Can Hyaluronidase Be Used?
There is no fixed waiting period. It can, in principle, be administered soon after an HA filler treatment or used to address a filler placed months or years earlier — the enzyme will act on hyaluronic acid wherever it finds it. Many practitioners nonetheless prefer to let recently placed filler settle for a period before assessing whether correction is genuinely needed, since some early puffiness or asymmetry resolves on its own as swelling subsides.
How Precise Is Correction With Hyaluronidase?
Hyaluronidase is well suited to addressing clear problems — visible asymmetry, lumping, or overcorrection — but it does not work with pinpoint accuracy. Tissue density and the resistance of the specific filler used both affect how evenly the enzyme dissolves HA in a given area, and it is possible to remove more filler than intended. Correcting a true problem and fine-tuning an acceptable but imperfect result are different undertakings, and the margin for error narrows considerably in the latter case.
Safety Considerations
Two safety questions come up consistently:
- Will it dissolve the patient's own hyaluronic acid? In principle, yes, though the effect is limited and temporary — the body continuously produces its own HA, and any depletion in the treated area typically resolves within a day or two as production catches up.
- Is there an allergy risk? Hyaluronidase is derived either from animal sources (commonly bovine or ovine) or produced as a recombinant human protein. Animal-derived formulations carry a somewhat higher reported allergy risk. Because hyaluronidase is also present in bee and wasp venom, patients with known allergies to those stings warrant particular caution, and a patch test is standard practice for first-time use in many clinics.
Re-Treatment: How Long Before New Filler Can Go Back In?
HA filler cannot be placed in the same session as hyaluronidase, but there is no single universal interval before re-treatment. Two things generally need to happen first: the enzyme needs to finish acting (its activity can continue for several days, so many practitioners wait a minimum of 48 hours), and any localised inflammation or swelling needs to resolve, which can take considerably longer. The appropriate interval is a matter of clinical judgement based on the individual case.
