Microneedling has become a staple of skin-quality practice, and it is increasingly paired with a regenerative active rather than used alone. Much of the online conversation focuses on "microneedling with exosomes" — but that pairing sits on shaky regulatory ground. Polynucleotides offer a more defensible route to the same goal: amplifying the skin's repair response through controlled injury plus a regenerative stimulus.
Why Combine at All
Microneedling works by creating thousands of controlled micro-channels in the skin. This triggers a wound-healing cascade — collagen induction — and, briefly, opens the skin to the delivery of topically applied actives. On its own it produces meaningful results for texture, pores and fine lines. The rationale for adding a regenerative agent is straightforward: the treatment already primes fibroblasts and repair pathways, so a complementary stimulus may extend that response.
Polynucleotides are a logical partner. As purified DNA fragments that support fibroblast activity, hydration and the tissue environment, they align with precisely the biology microneedling sets in motion.
The Devices
Microneedling is delivered with several tools, each suited to different settings:

The right sequence, depth and timing are clinical decisions for the treating practitioner and depend on the device, the product's licensed use and the patient. Injectable polynucleotides have their own placement technique and should not be assumed to be interchangeable with topical application over needled skin.
Why Polynucleotides Rather Than Exosomes
The "microneedling plus exosomes" trend deserves a clear-eyed note. No exosome product holds FDA approval for aesthetic use, and regulators have flagged concerns about unapproved exosome products applied to or injected into skin. Presenting exosomes as an established, approved add-on is not accurate. Polynucleotides, by contrast, are CE-marked injectables with a defined regenerative rationale — a more compliant and better-characterised choice for practitioners who want an evidence-aware protocol. Exosomes remain worth understanding as context, but they are not a substitute we can responsibly recommend or supply.
Aftercare and Expectations
Combining treatments does not change the fundamentals: transient redness, swelling, pinpoint bleeding and small papules are commonly reported and generally settle within a few days. Sun protection and a gentle post-procedure regimen are standard. As with any regenerative approach, results build over a course rather than appearing after a single session, and no specific outcome can be guaranteed.
Patient Selection and Cautions
The combination is not appropriate for everyone. Microneedling should be avoided over active acne, infection, or inflamed or broken skin, and standard injectable exclusions — pregnancy and lactation, bleeding disorders, and relevant allergy history including fish allergy for the polynucleotide component — apply to the injectable side. Patients prone to post-inflammatory pigmentation need careful counselling and sun avoidance afterwards. It is also worth being clear about the difference between clinic and home use: at-home rollers operate at shallow depths and are not a route for delivering prescription-style actives into the skin. Meaningful combination protocols belong in trained hands, where depth, device hygiene and product placement can be controlled.
