Tear Trough Filler: Anatomy, Causes and Product Selection

The tear trough — the groove running from the inner corner of the eye down toward the cheek — is one of the more technically demanding areas to treat, and one of the most requested. Patients rarely use the clinical term; they ask about "the shadows under my eyes" or say they look tired no matter how much they sleep. Understanding the anatomy behind that appearance is what allows a practitioner to judge whether filler is likely to help.

What the Tear Trough Is and Why It Forms

The tear trough sits at a natural transition point where the thin skin of the lower eyelid meets the thicker skin of the cheek, and where the underlying fat pads and bony orbital rim create a visible groove even in younger faces. Several factors combine to make it more pronounced:

  • Thin overlying skin. The eye contour area has some of the thinnest skin on the face, making the darker colour of underlying blood vessels more visible.
  • Fat pad and bone anatomy. Some patients are born with less fat volume in this area, or a more recessed orbital rim, producing a deeper hollow independent of age.
