Recognising It Early

Early recognition is the single most important skill, because the window to act is short. Warning signs during or shortly after injection include:

  • Disproportionate pain — pain out of keeping with the procedure, though occlusion can also be relatively painless, especially under anaesthetic
  • Blanching — an immediate white or pale change in the skin as flow is cut off
  • Dusky or mottled discolouration — a livedo, reticulated or blue-grey pattern appearing over minutes to hours
  • Delayed capillary refill and coolness of the affected skin
  • Any visual disturbance, severe headache or ocular pain — a red-flag scenario demanding immediate escalation

The deceptive part is timing: some signs appear instantly, others evolve over hours. Any suspicion warrants action rather than watchful waiting.

Why "Time Is Tissue"

The phrase captures the core reality: the longer perfusion is absent, the more tissue is lost, and the harder reversal becomes. Management is therefore not something to schedule for tomorrow — it begins the moment occlusion is suspected. Hesitation, or sending a patient home to "see how it settles," is how a recoverable event becomes a permanent scar.

The Cornerstone: Hyaluronidase

For HA fillers, the enzyme hyaluronidase is the cornerstone of emergency management. Established emergency protocols use high, repeated doses over the affected area, with reassessment and re-dosing until perfusion returns — this is a protocolised, escalating response, not a single token injection. Every clinic should have a written emergency plan, a stocked and in-date kit, a defined route to urgent hospital or ophthalmology care for suspected visual compromise, and staff who have rehearsed the sequence. Documentation and follow-up complete the picture. Preparation is not optional paperwork; it is the difference between a good outcome and a catastrophic one.

Prevention Lowers the Odds

Emergency readiness is essential, but prevention is where most of the risk is managed. The measures taught in accredited training — sound knowledge of facial vascular anatomy, careful product and needle-versus-cannula selection, aspiration where appropriate, slow injection under low pressure, keeping the needle tip moving, and depositing in small increments — all reduce the chance of an intravascular event in the first place. None of them eliminates the risk, which is why the enzyme and the protocol still matter, but careful technique means the emergency plan is called upon far less often. Prevention and preparation are two halves of the same responsibility, not alternatives.
