As elective filler dissolving has grown from an emergency measure into a routine request, a practical question has moved to the centre of clinic discussions: should hyaluronidase be spot-tested for allergy before use? The enzyme is derived from animal sources, allergic reactions are recognised, and a rising volume of elective dissolving means more patients are being exposed. The answer is not a simple yes or no — it depends heavily on whether the setting is elective or an emergency. Why the Question Has Grown
Two shifts have pushed allergy testing up the agenda. First, the sheer increase in elective dissolving — for migration, overfilling and "reset" requests — means the enzyme is now used far more often, and on healthier, lower-urgency patients who have time for precautions. Second, awareness of hyaluronidase hypersensitivity has risen, including both immediate reactions and delayed swelling that can be mistaken for something else. When a drug is being used more, and electively, the threshold for caution naturally shifts.
The Case for Spot Testing Before Elective Use
Proponents make a reasonable argument in the elective context. When a procedure is planned and non-urgent, there is time to reduce the risk of an unexpected reaction, and a preliminary test may flag a sensitised patient before a larger exposure. The argument is strongest for patients with a history of atopy, multiple allergies, or a prior reaction to the enzyme, where the individual risk profile is higher. In this framing, a pre-test is simply proportionate caution for an elective, off-label use.
Many practitioners who dissolve electively therefore consider a preliminary test as part of their consent and safety process, particularly in higher-risk patients.
The Limits of Testing
The counter-arguments are equally serious and are why this remains a debate rather than a settled rule:
- Imperfect predictive value. A negative preliminary test does not guarantee a later dose will be tolerated, and testing can produce results that are difficult to interpret. It reduces uncertainty; it does not remove it.
- Delayed reactions. Some hypersensitivity to hyaluronidase is delayed, appearing hours or days later, which a single immediate-read test will not capture.
- The risk of the test itself. Introducing the enzyme to sensitise or provoke a reaction is not zero-risk, and repeated exposure carries its own theoretical concerns.
- Practicality and consistency. Protocols vary between clinics, and an inconsistently applied test can offer false reassurance rather than genuine safety.
None of this makes testing worthless in the elective setting — it makes it one input into clinical judgement, not a guarantee.
Emergencies Are a Different Question Entirely
Here the debate narrows to a clear point of consensus. In a suspected vascular occlusion, hyaluronidase is the time-critical drug that can save tissue, and treatment must not be delayed to perform an allergy test. "Time is tissue" governs the emergency, and the small risk of a reaction to the enzyme is accepted against the far greater risk of tissue loss. Any allergy-testing policy a clinic adopts for elective work must state explicitly that it does not apply in an emergency. Confusing the two is the single most dangerous mistake in this area.

Patients should know that the enzyme can affect their own native hyaluronic acid as well as the filler, and that reactions, though uncommon, are managed as part of proper practice.
