Microneedling and platelet-rich plasma (PRP) are each established tools for stimulating the skin's own repair response, and pairing them is one of the more studied combination treatments in aesthetic medicine. Understanding why the two are used together — rather than treating either as a standalone procedure — is useful both for explaining the treatment to patients and for positioning it correctly within a clinic menu.
What Microneedling Does
Microneedling, also referred to as collagen induction therapy, works by creating controlled micro-punctures in the skin using fine needles. These micro-injuries prompt an inflammatory response and the release of growth factors, which in turn encourage fibroblast activity and the formation of new collagen and elastin in the dermis. The two most common delivery devices are dermarollers, which use a wheel of fixed needles, and motorised microneedling pens, which allow the practitioner finer control over needle speed, size and penetration depth. Needle depth is typically adjusted according to the treatment area and concern, with finer, shallower settings used on thinner skin such as around the eyes, and deeper settings reserved for thicker-skinned areas or more pronounced scarring.
What PRP Contributes
PRP is prepared by centrifuging a small sample of the patient's own blood, which separates it into components and yields a plasma fraction with several times the platelet concentration of whole blood. Platelets are rich in growth factors that promote cell division and support the same repair pathways that microneedling activates on its own. Because PRP is autologous — derived from the patient's own blood — it carries a favourable safety profile with respect to allergic reaction to the material itself.
Why the Two Are Often Combined
The micro-channels created during microneedling temporarily increase the skin's permeability to substances applied afterwards. Applying PRP topically once those channels are open allows the growth factors it contains to reach deeper into the dermis than topical application alone would achieve. In this way, the mechanical stimulus from microneedling and the biological stimulus from PRP work on the same repair pathway rather than two separate ones, which is the rationale most often given for combining the treatments. Some studies describe absorption of topically applied active ingredients increasing substantially once these channels are open, though the exact figure varies by study and by the specific active ingredient being measured.
Topical Versus Intradermal PRP Application
PRP can be introduced in more than one way once the microneedling pass is complete. Some practitioners apply it as a topical layer immediately after treatment, allowing it to penetrate through the freshly created micro-channels. Others inject it intradermally as a distinct step, either instead of or in addition to topical application. The choice between approaches is a matter of practitioner training and preference, and both are described in the literature as supporting the same underlying repair mechanism.
Indications Commonly Associated With the Combination
Microneedling with PRP is most often discussed in relation to:
- Fine lines and early signs of ageing.
- Acne scarring and other textural irregularities.
- Hyperpigmentation and uneven skin tone.
- Sun damage and enlarged pores.
- Periorbital concerns, including the appearance of the under-eye area.

What Patients Should Expect Afterwards
Mild redness and swelling are common in the first day or two and generally resolve on their own. Practitioners typically advise keeping the treated area clean, avoiding direct sun exposure, and using a high-SPF sunscreen if sun exposure is unavoidable, for the first few days. Minimising the use of other topical products and avoiding activities that induce heavy sweating, such as saunas or intense exercise, is also commonly advised during that initial recovery window. As with most collagen-induction treatments, results build over a short course of sessions rather than a single visit. A typical course involves several sessions spaced a few weeks apart, since both microneedling and PRP rely on a cumulative repair response rather than delivering their full effect after a single treatment.
