PDO vs PCL vs PLLA Threads: Materials Compared

When practitioners compare threads, the conversation often fixates on shape — mono, cog, screw — but the material the thread is made from is just as consequential. PDO, PCL and PLLA are all absorbable, but they break down at very different rates, and that single property shapes how long a result is likely to last and how the tissue responds along the way.

Three Materials, Three Timelines

All three are established resorbable polymers with a history in surgical suturing. The practical difference is absorption speed.

PDO (polydioxanone)

PDO is the most widely used thread material and the reference point for the category. It has a long track record as an absorbable suture and is typically broken down over a period of months, prompting a tissue response as it goes. It is available across the full range of geometries — smooth mono threads, barbed cog threads for lifting, and screw designs for localised support — which makes PDO the versatile default for most thread work. Its relatively shorter absorption is not a weakness so much as a characteristic: results are real but temporary, and maintenance is expected.

PCL (polycaprolactone)

PCL has the slowest absorption of the three commonly cited materials, which is why it is associated with a more prolonged presence and a longer-horizon tissue response. In practice, pure-PCL threads are less common in general catalogues than PDO, and caprolactone often appears as part of a copolymer. The Relife Definisse Revitalise thread, for example, is made of poly(L-lactide-co-caprolactone) — a blend that combines lactide and caprolactone chemistry to extend the absorption window beyond that of standard PDO.

PLLA (poly-L-lactic acid)

PLLA absorbs more slowly than PDO and is strongly associated with a gradual, collagen-stimulating response — the same biostimulatory reputation PLLA carries as an injectable. It is the material behind well-known cone-based lifting systems such as Silhouette Soft, where bidirectional cones anchor and reposition tissue while the material stimulates over time. PLLA threads are typically discussed in the context of a longer, more regenerative horizon than PDO.

What This Means for Longevity

It is tempting to rank the materials purely by how long they last, but the honest picture is more layered.

  • Slower absorption generally means a longer-lasting presence, so PCL and PLLA are broadly associated with more prolonged results than PDO, all else being equal.
  • Longevity is not just about the polymer. Thread geometry, the number placed, the region, tissue quality and individual metabolism all influence how long a visible result holds. Figures should be framed as typical ranges, not promises.
  • The material also shapes the character of the response, not only its duration — PLLA in particular is discussed for its gradual, stimulatory behaviour rather than a purely mechanical one.

So the material sets the timeline and the flavour of the tissue response; it does not, on its own, dictate the outcome.

Matching Material to Goal

  • Versatile, well-understood, shorter-horizon support and lift → PDO, which also offers the widest choice of geometries for mixing within a plan.
  • A longer absorption window and more prolonged presence → a caprolactone-containing (PCL-type) thread such as a P(LA-CL) blend.
  • A gradual, biostimulatory, longer-lasting lift → a PLLA cone-based system.

In many plans the pragmatic answer is still PDO for its flexibility, with the slower-absorbing materials chosen where a longer horizon justifies them and the practitioner is trained in their use.

Realistic Expectations and Safety

Whatever the material, the fundamentals of thread work do not change:

  • Results are visible but modest, suited to appropriate, limited laxity rather than significant descent — where surgery is indicated, thread material choice will not close the gap.
  • The effect is temporary, because every one of these polymers is designed to absorb.
  • It is a clinical procedure with transient swelling, bruising or discomfort, and it belongs in trained hands with clearly specified, sterile-packaged product. This article is educational and does not describe an insertion technique.
